Attendance & Cancellation Policy
To ensure consistent, quality care, Myndset Therapeutics maintains a clear attendance and cancellation policy. Clients are asked to provide at least 24 hours’ notice for cancellations to avoid a fee. Late cancellations may incur a $50 charge, and no-shows are subject to a $75 fee. Repeated missed appointments may result in higher charges and the suspension of future sessions until outstanding balances are resolved. Our goal is to protect both your time and your therapist’s. Forensic Fees & Court Appearance Policy
While therapeutic services at Myndset Therapeutics are not intended for legal proceedings, there may be cases in which a therapist is subpoenaed or asked to participate in court-related matters. Our Forensic Fees Policy outlines rates for court preparation, testimony, depositions, document filing, and related legal involvement. Records Request & Retrieval Policy
In accordance with Michigan law (MCL 333.26269), Myndset Therapeutics may charge a fee for medical record retrieval and duplication. Requests are typically fulfilled within 7 business days. Fees include a $20 retrieval charge plus per-page copy rates, with additional charges for non-paper formats, shipping, and records older than 7 years. Patient Account Balance Policy
The Patient Account Balance Policy outlines expectations for managing copays, deductibles, and private pay balances. Clients are responsible for keeping their accounts in good standing, with copays due after each session and deductibles payable upon claim processing. If a balance exceeds $250 or remains unpaid past the start of a new insurance year, services may be paused until resolved. Private pay clients are billed securely through IvyPay. Telehealth is a convenient and secure way to receive therapeutic support when meeting in person is not possible. Our Consent for Telehealth Consultation form outlines the benefits, risks, limitations, and expectations associated with virtual sessions. It ensures that both therapist and client understand the use of technology, confidentiality considerations, and emergency protocols related to remote care.
